Job Responsibilities – you knew they were coming!
- Provide treatment and patient care according to state and federal regulations and professional guidelines
- Perform appropriate assessment of physical, social, and psychological status
- Use assessment information and critical thinking skills to develop an individualized plan of care; revise and reassess changes as appropriate
- Actively seek patient, family, team, and physician involvement to develop plan of care, including needed education for patient and/or caregivers
- Set priorities, coordinate, and organize patient care delivery through effectively managing time, supplies, and resources
- Initiate referrals as needed to other team members and maintain open and ongoing communication with peers to ensure patient needs are met
- Maintain patient records showing systematic assessment, planning intervention, and evaluation
- Participate in team meetings and coordinate patient care with other members of the team
Skills and Qualifications – we know you have them!
- Current Registered Nurse (RN) license in the state of practice
- Home health/hospice experience as a Registered Nurse (RN) preferred
- Ability to utilize full range of movement to transfer patients
- Ability to communicate effectively both verbally and in writing
- Valid driver’s license, clean driving record, and reliable automobile
- Proof of current automobile insurance
- Criminal background check clearance/fingerprint clearance
- Current CPR certification
- Negative TB test
- Other state required tests and/or examinations
Physical Requirements – to uplift others!Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet essential duties of this position.Mental Requirements – you went to school for this!Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.Working Environment – it’s a change in scenery!Works primarily in patient homes/care facilities with some work being completed in office; travel to and from patients is required. You must be able to tolerate exposure to several elements, including but not limited to the following: odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
