Directs admission process between physicians, other referral sources, and the facility staff to efficiently evaluate and admit residents. Develop and implements marketing strategies for referral source development.Essential Job Functions1. Screen/assess appropriate referrals for admission and discuss potential for admission with the staff.2. Direct and process admissions and serve as the liaison between the Directors of Admissions, hospitals, admitting/attending physicians, and other referral sources.3. Develop a system for coordinating and monitoring resident assessments and referrals.4. Conduct and document nursing assessments as necessary.5. Complete and submit admission assessment reports as requested.6. Coordinate admissions with necessary staff to ensure a smooth flow of information and activity.7. Serve as a point-of-contact for referrals/inquires and conduct initial clinical review as indicated.8. Process referrals and inquires.9. Work with other facility departments to ensure an accurate and timely admission process.10. Maintain an accurate record of available beds/units.11. Communicate the care needs and medical equipment needs of residents upon admission with the Director of Admissions and assist as indicated.12. Conduct rounds and provide information to appropriate staff to ensure readiness for admissions.13. Obtain necessary medical records for admission, i.e., diagnosis, physician orders, OBRA screening, etc.14. Assume additional responsibilities and special projects as assigned or needed.Responsibilities1. Ensure admission assessments and orders are obtained, physician to physician communication occurs where necessary, and that the resident’s admission assessment process is managed efficiently and effectively.2. Ensure that the resident/family has an understanding of level of care, expectations.3. Ensure that information about resident progress is communicated as needed with staff, physicians, and referral sources.4. Ensure good public relations with the general public, residents, families, and guests, to promote a positive image.5. Ensure that residents and resident representatives/family members are oriented to payment source/rates as appropriate, billing procedures, visitor/guest privileges, restrictions, and resident care procedures as appropriate.6. Treat all residents fairly, with kindness, dignity, and respect.7. Work with residents, physicians, nursing staff in a polite, courteous, and professional manner at all times and provide support to residents and their families.8. Maintain a clean and safe work environment.9. Maintain confidentiality of resident and organization information and respect residents’ privacy and dignity.10. Cooperate with other staff members to achieve departmental and organizational goals; promote good inter/intradepartmental relations.11. Act as a role model for good facility customer service.12. Adhere to State and Federal regulations governing Long Term Care.13. Participate in staff development programs and staff meetings.14. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.15. Comply with organizational policies and procedures.16. Report unsafe/hazardous conditions and/or defective equipment immediately.17. Maintain a neat, professional appearance and adhere to dress code.18. Comply with Corporate Compliance Program.19. Participate in facility and divisional meetings as requested.20. Report all allegations of resident abuse, neglect and/or misappropriation promptly to Executive Director.EducationMust have successfully completed an accredited Registered Nurse or Licensed Practical Nurse program and be currently licensed as a nurse by the State of Alabama.Experience Minimum of two (2) year’s experience in case management and discharge planning.
